    目的:探讨循环雌酮(Estrone,E1)、2-羟雌酮(2-hydroxyestrone,2-OHE1)、16α-羟雌酮(16α-hydroxyestrone,16α-OHE1)水平及2-OHE1∶16α-OHE1比值与绝经后乳腺癌的相关性。方法:采用ELISA法检测绝经后40例乳腺癌、21例乳腺纤维腺瘤和19例乳腺病患者循环E1、2-OHE1和16α-OHE1水平并计算2-OHE1∶16α-OHE1比值。结果:3组患者循环E1、16α-OHE1水平和2-OHE1∶16α-OHE1比值差异无统计学意义(P>0.05),3组患者2-OHE1水平差异有统计学意义(P<0.05),乳腺癌组较乳腺病组低,差异有统计学意义(P<0.05)。循环E1、2-OHE1、16α-OHE1水平和2-OHE1∶16α-OHE比值与雌激素受体(Estrogen receptor,ER)、人表皮生长因子受体2(HER2)情况、腋窝淋巴结转移情况以及肿瘤大小无关(P>0.05)。乳腺癌危险性与2-OHE1∶16α-OHE1比值无相关性[OR,0.828;95%CIfor exp(B),0.425~1.611;P=0.578],与2-OHE1呈负相关性[OR,0.418;95%CI for exp(B),0.180~0.971;P=0.043]。结论:实验结果提示2-OHE1水平与乳腺癌危险性呈负相关,未发现2-OHE1∶16α-OHE1比值预示乳腺癌危险性。

    Abstract:

    Objective:To study the correlation among circulating estrone (E1), 2-hydroxyestrone(2-OHE1), 16α-hydroxyestrone(16α-OHE1), the ratio of 2-hydroxyestrone to 16α hydroxyestrone(2-OHE1∶16α-OHE1) and breast cancer in postmenopausal women. Methods:Serum levels of E1,2-OHE1,16α-OHE1 were measured by ELISA method in 40 cases of breast cancer,21 cases of breast fibroadenoma and 19 cases of mastopathy in postmenopausal women; the ratio of 2-OHE1 to 16α-OHE1 was calculated. Results:There were no significant difference in circulating levels of E1,16α-OHE1 and the ratio of 2-OHE1 to 16α-OHE1 in three groups (P>0.05). The levels of 2-OHE1 were significantly different in three groups(P<0.05); it’s level in breast cancer group was significantly lower than that in mastopathy group(P<0.05). Circulating levels of E1,2-OHE1,16α-OHE1 and the ratios of 2-OHE1 to 16α-OHE1 were not associated with ER, HER2, axillary lymph node status or tumor size (P>0.05). Breast cancer risk was not associated with the ratio of 2-OHE1 to 16α-OHE1(Odds ratio was 0.828, 95% confidence interval was 0.425~1.611, P>0.05),but it was negatively associated with the level of 2-OHE1(Odds ratio was 0.418, 95% confidence interval was 0.180~0.971, P<0.05).Conclusion:The results suggest that the level of 2-OHE1 is negatively associated with breast cancer risk, and do not support that the ratio of 2-OHE1 to 16α-OHE1 predicts breast cancer risk.
