不同年龄阶段恶性肿瘤儿童行为问题及影响因素的相关研究

Analysis on behavior problems of children with malignant tumor at various ages and their relevant influencing factors

摘要:

目的：探讨不同年龄阶段恶性肿瘤儿童心理和行为特点及影响因素，为临床心理行为干预治疗提供依据。方法：采用Rutter行为问题父母问卷，对120名恶性肿瘤儿童分别实施问卷调查，同时采用临床观察、深入访谈等方式确定影响恶性肿瘤儿童行为问题的主要因素。结果：120名恶性肿瘤儿童中，行为问题检出率：0~3岁54.6%，≤7岁61.5%，≤14岁24.3%，显著高于对照组（10.7％），差异有统计学意义（P<0．01）。结论：家长的教养方式、家庭关系、父母及儿童的应对方式、主要照护人的选择等是恶性肿瘤儿童行为问题形成的主要因素。医护人员应有针对性地进行心理疏导和治疗，以预防和矫治不良行为，提高心理应对能力，培养良好性格，促进恶性肿瘤儿童身心健康发展。

Abstract:

Objectives：To explore psychological and behavior patterns of children with malignant tumor at various ages as well as their influencing factors in order to provide references for clinic treatment. Methods：Totally 120 children with malignant tumor were in－vestigated with Rutter behavior questionnaire. Together with clinic examination and intensive talks，the major influencing factors were clarified. Results：Among the 120 children，detection rate of behavior problem was significantly higher in 54.6% aged under three，61.5% aged between four to seven，24.3 aged between eight to fourteen than in control group（10.7%），with statistical significances（P＜0.01）. Conclusions：Major influencing factors for behavior problems of children with malignant tumor include education methods of parents，family intimacy，responses of both children and parents to tumors，selection of care-taker. Besides，individual psychological and physical treatment should be provided to enhance their responding ability and cultivate their characteristics，ultimately promote their sound development.
