    • Research progress of high mobility group box-1 in acute lung injury/acute respiratory distress syndrome

      Abstract:Acute lung injury(ALI) and acute respiratory distress syndrome(ARDS) are life-threatening clinical syndromes in the in-tensive care unit. At present,ARDS mortality rate remains high(>40%). Its treatment and the use of biological markers for early di-agnosis are still major problems in the field of critical care medicine. High mobility group box-1 protein(HMGB1) has recently been identified as a new proinflammatory cytokine and late mediator of inflammation. It is associated with sepsis,cancer,autoimmune dis-eases and many other diseases. HMGB1 is an alarmin its concentration can reflect the severity of the inflammatory and tissue injury,and is closely related with the prognosis of the disease. Antagonizing HMGB1 can block the cascade of inflammatory pathways,which may provide a new therapeutic target for the treatment of ARDS.In this paper,we reviewed on the role of HMGB1 in ALI/ARDS.

    • Study of transcriptional regulation mechanism of kfuABC operon by cAMP receptor protein in Klebsiella pneumoniae

      Abstract:Objective:To investigate the transcriptional regulation mechanism of kfuABC operon by cAMP receptor protein(CRP) in Klebsiella pneumoniae. Methods:Real-time PCR was firstly used to validate the operon structure of kfuABC by using the intergenic re-gion spanning primers. Primer extension assay was employed to detect the transcriptional start site of kfuA by using the total RNAs of wild type(WT) as template. The entire promoter-proximal region of kfuA was amplified and cloned into the plasmid pHRP309 con-taining a promoterless lacZ reporter gene. The recombinant LacZ reporter plasmid kfuA∷pHRP309 was transformed into the crp null mutant(?驻crp) and WT,respectively,to measure the promoter activity(the β-Galactosidase activity) of kfuA by using the β-Galac-tosidase enzyme assay system. Then,total RNAs were extracted from ?驻crp and WT strains,and the quantitative Real-time PCR was carried out to calculate the transcriptional variation of kfuA in the ?驻crp and WT. The over-expressed His-CRP was purified under native conditions with nickel loaded HiTrap Chelating Sepharose columns(Amersham),and the entire promoter-proximal region of the kfuA gene was amplified by PCR from WT strain. Then,the electrophoretic mobility shift assay and DNaseⅠ footprinting experiment were applied to analyze the DNA-binding site of His-CRP to kfuA promoter region in vitro. Results:The transcription of kfuABC operon was regulated by CRP negatively. Primer extension assay detected only one transcriptional start site. The DNaseⅠ footprinting assays showed that the binding site was located from 204 bp to 171 bp upstream of kfuABC. Conclusion:The transcription of kfuABC oper-on is repressed by CRP via directly combination with promoter region of kfuABC operon in Klebsiella pneumoniae.

    • Effects of mycobacteriophage Chy3 on growth and function of human airway epithelial cell 16HBE

      Abstract:Objective:To explore the effects of different titers of mycobacteriophage Chy3 on growth and function of human airway ep-ithelial cell 16HBE. Methods:Inverted microscope was used to observe cellular morphology after applying different titers of mycobac-teriophage Chy3. Cell viability and apoptosis rates were analyzed by CCK-8 assay and flow cytometry. Additionally,the secretion lev-els of IL-6/IL-8 in cell culture supernatant were detected by ELISA. RT-PCR and Western blot were performed to detect MUC5AC mRNA and protein expression. Results:There was no difference in cellular morphology,cell viability,apoptosis rate,secretion levels of IL-6/IL-8,MUC5AC mRNA and protein expression between cells treated with different titers of Chy3 and normal controls(P >0.05). Conclusion:Neither high nor low titer of mycobacteriophage Chy3 exerts effects on growth and function of human airway epithelial cell 16HBE in vitro,therefore mycobacteriophage has better security.

    • Expression of NF-κB and Bak in oxidative stress-induced apoptosis of alveolar type Ⅱ epithelial cells

      Abstract:Objective:To investigate the changes of nuclear factor-κB(NF-κB) p65 and Bak expression during the apoptosis of type Ⅱ(ATⅡ) epithelial cells induced by oxidative stress,and the apoptotic outcomes of ATⅡ cells as well. Methods:Primary ATⅡ cells were separated,purified and cultured from adult Sprague-Dawley rats treated with 500 μmol/L hydrogen peroxide(H2O2) for different periods of time,with the purpose of establishing an apoptotic cell model by oxidative injuries. The change of cell morphology was ex-plored by phase contrast microscope. Apoptotic ratio was measured by flow cytometry. The changes in expression of NF-κB p65 and Bak were investigated by Western blot. The location of NF-κB p65 was detected by immunofluorescence and observed under laser con-focal microscopy. Results:After exposure to H2O2 for 3 h,ATⅡ cells turned smaller,with karyopycnosis and reduction of cytoplasmic granules. The cell apoptotic ratios after exposure to H2O2 for 1 h and 3 h were both increased obviously(P=0.000,P=0.000),compared with those of normal control group. Western blot detected that the nucleic expression of NF-κB p65 and the expression of Bak after exposure to H2O2 for 3 h were increased remarkably compared with those of normal control group(P=0.000,P=0.000). The nucleic translocation of NF-κB p65 after H2O2 treatment was also increased in the merged images under laser confocal microscopy. Conclusion:High level of oxidative stress induces ATⅡcells apoptosis and up-regulates the nuclei expression of NF-κB p65 and the expression of Bak.

    • Effect of Center-Supplementing and Qi-Boosting Decoction combined siRNA on LRP expression of A549/DDP cell

      Abstract:Objective:To discuss the effect of concomitant application of small interfering RNA(siRNA) after targeted resection of lung resistance protein(LRP) and Buzhongyiqitang serum on the human lung adenocarcinoma drug-resistant cell-line A549/DDP. Methods:The siRNAs which had the same sequences of LRP gene were designed and synthesized,then the human lung adenocarcinoma drug-resistant cell-line A549/DPP was transected with transfection reagent. The experimental cells were grouped into control group,Buzhongyiqitang serum group,siRNA-processed group and Buzhongyiqitang plus siRNA-processed group. RT-PCR was applied to de-termine the mRNA expression of LRP. Western blot and immunocytochemical method were adopted to determine the protein expres-sion of LRP. Results:Protein and mRNA expression of LRP in Buzhongxiqitang serum group,siRNA group and Buzhongyiqitang plus siRNA-processed group was decreased significantly compared with that in control group(P<0.05). Conclusion:Both Buzhongyiqitang serum and siRNA serve effectively in decreasing protein expression of LRP,and Buzhongyiqitang serum combined siRNA show syner-gistic effect.

    • Preparation of two kinds of superparamagnetic iron oxide nanoparticles and their acute toxicity to rat lungs

      Abstract:Objective:To prepare superparamagnetic iron oxide nanoparticles(SPIONs) modified by chitosan and sodium oleate and to investigate their acute lung toxicity. Methods:The SPIONs were prepared by coprecipitation and their properties were measured. Fifty-four SD rats were divided into three groups randomly:control group(n=18),chitosan-SPIONs group(n=18) and sodi-um oleate-SPIONs group(n=18). The rats were exposed to SPIONs by intratracheal instillation. General condition of the rats was observed. Six rats of each group were killed randomly at 24 h,72 h and 14 d after the exposure. The major blood biochemical indicators were analyzed with an automatic biochemical analyz-er. Bronchoalveolar lavage fluid(BALF) was taken and the total cells and neutrophil cells were counted. The pathological changes of lungs were observed with HE staining. Results:①Both of the SPIONs exhibited structure of spherical crystallization with a diameter of 30 nm. ②The normal situation of rats was good and no rat died;③Only two blood biochemical indicators including total protein(TP) of 24 h and alkaline phosphatase(ALP) of 14 d were different from those of control group(TP(g/L) 24 h group:chitosan-SPIONs group(n=6):62.40±3.60,sodium oleate-SPIONs group(n=6):69.95±5.16,control group(n=6):63.22±2.55,F=6.344,P=0.010;ALP(u/L) 14 d group:chitosan-SPIONs group(n=6):78.83±7.92,sodium oleate-SPIONs group(n=6):106.83±26.95,control group(n=6):99.85±9.63,F=4.336,P=0.033). ④Total cells(×104) in BALF of experimental groups were higher than those in control group at 24 h and 72 h and total cells of sodium oleate-SPIONs group were obviously higher than those of chitosan-SPIONs group(chitosan-SPIONs 24 h group(n=6):780.00±130.58,sodium oleate-SPIONs 24 h group(n=6):1 000.00±166.85,control 24 h group(n=6):498.33±114.75,F=19.605,P=0.000;chitosan-SPIONs 72 h group(n=6):809.17±197.49,sodium oleate-SPIONs 72 h group(n=6):920.00±125.54,control 72 h group(n=6):580.00±123.29,F=7.735,P=0.005). Neutrophil cells in BALF were more in sodium oleate-SPIONs group than in chitosan-SPIONs group.⑥HE staining showed the fracture of pulmonary alveoli,thickening of alveolar walls,and inflammatory cell infiltration in the experimental groups which were more serious in sodium oleate-SPIONs group than in chitosan-SPIONs group. Conclusion:Chitosan modified SPIONs have less toxicity to the rats’ lungs than sodium oleate modified SPIONs and their lung toxicity reduces over time.

    • Experimental study of inhaling budesonid in treating pulmonary fibrosis in rats and the possible mechanism

      Abstract:Objective:To investigate the effects and the possible mechanism of inhaling budesonid in treating bleomycin-induced pul-monary fibrosis in rats. Methods:Forty-five SD rats were randomly divided into three groups(15 in each group):control group,model group and intervention group. Rats in intervention group inhaled budesonid every two days from the 7th d after intratracheal instilla-tion while those in model group and control group inhaled the same volume of 0.9% normal saline. Five rats in each group were ran-domly killed on the 7th,14th,28th d after atomizing inhalation and the pulmonary tissues were preserved respectively. HE and Masson staining were used to determine the extent of alveolus inflammation and pulmonary fibrosis. Immunohistochemical technique was used to evaluate the level of transforming growth factor-β(TGF-β) and connective tissue growth factor(CTGF) in lung tissues. RT-PCR was used to detect the level of type Ⅰ and Ⅲ collagen in pulmonary tissues. Results:Pulmonary tissue pathology showed that the ex-tent of alveolus inflammation and pulmonary fibrosis was less intense in control group than in model group and intervention group and was better in intervention group than in model group. Significant over-expressions of CTGF and TGF-β were shown in pulmonary tissues of rats in model group and intervention group(higher expressions in model group than in intervention group,P<0.05) compared with those in control group(P<0.05). Expressions of type Ⅰ and Ⅲ collagen were significantly higher in model group than in con-trol group(P<0.05),but lower in intervention group than in model group(P<0.05). Conclusion:Budesonid has anti-fibrosis po-tency,the mechanism of which may be reducing the production of type Ⅰ and Ⅲ collangen through inhibiting the expressions of cy-tokines such as TGF-β and CTGF.

    • Expression of pre-B-cell colony enhancing factor and its influence on the expression of PBEF,TNF-α,IL-1β and NF-κB p65 in pulmonary tissues of rats with ARDS

      Abstract:Objective:To investigate the influence of pre-B cell colony enhancing factor(PBEF) on the mRNA expression of PBEF,tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β) as well as the protein expression of nuclear factor-κB(NF-κB)p65 in lung tissue in rats with acute respiratory distress syndrome(ARDS) induced by oleic acid tail vein injection and drug intervention;to po-tentially verify the pro-inflammation role of PBEF in ARDS and to provide new ideas and evidences for further study of the patho-genesis,pathophysiological processes and treatment of ARDS by referring to the phenomena observed by other scholars in vitro. Methods:Forty healthy adult male SD rats were randomly divided into control group(group C),model group(group OA),drug intervention group(group D),and solvent control group(group S). Rats in group OA,group D and group S were used to induce ARDS models by oleic acid tail vein injection at a dose of 0.15 ml/kg. Rats in group D received PBEF inhibitor FK866 10 mg/kg and group S also received the same volume of its solvent dimethyl sulfoxide by intraperitoneal injection. Whether the models were successfully made was preliminary determined by rats’ respiratory rates,color of skin and lips as well as their activity. Rats were anes-thetized by intraperitoneal injection of 3.5% chloral hydrate(10 ml/kg) six hours after successfully modeling. The right upper lobe of lung was immediately doused in formalin for im-munohistochemical detection and the right lower lobe was immediately frozen at -80 ℃ refrigerator or liquid nitrogen for detecting the expression of PBEF,TNF-α,IL-1β mRNA by RT-qPCR,as well as the protein expression of NF-κB p65 by Western blot. Results:The protein expression of PBEF could be found in the bronchial epithelia,vascular endothelia,alveolar wall and alveolar edema fluid of rats with ARDS by immunohistochemical detection. The expression of PBEF,TNF-α,IL-1β mRNA and NF-κB p65 protein significantly increased in all groups except group C(PBEF:P=0.000,P=0.000,P=0.000;TNF-α:P=0.035,P=0.000,P=0.000;IL-1β:P=0.000,P=0.000,P=0.000;NF-κB p65:P=0.000,P=0.000,P=0.000). Rats in group D had an ameliorated change in the expression of inflammatory factors mRNA and NF-κB p65 protein compared with those in group OA and group S(PBEF:P=0.002,P=0.006;TNF-α:P=0.004,P=0.001;IL-1β:P=0.001,P=0.015;NF-κB p65:P=0.001,P=0.000). Conclusion:PBEF may induce the inflamma-tory injury of pulmonary tissues by activating the inflammatory response and promoting the expression of inflammatory cytokines.Thus it may play an important role in the development of ARDS.

    • Regulation mechanism of MAP kinases on aquaporin 5 mRNA and protein expression in hyperoxic lung injury

      Abstract:Objective:To investigate regulation mechanism of mitogen activated protein kinases(MAPKs) signaling pathway on Aqua-porin 5(AQP5) expression disorder in hyperoxic lung injury(HLI). Methods:Forty-eight Wistar rats aged three weeks old were ran-domly divided into 8 groups(n=6):room-air group(A),hyperoxia group(H),hyperoxia + ERK inhibitor PD98059(H+PD),hyperoxia + p38 inhibitor SB203580(H+SB),hyperoxia + JNK inhibitor SP600125(H+SP),and the corresponding inhibitors control groups(A+PD,A+SB,A+SP). The rats in hyperoxia groups were kept in cabinet with high concentration of oxygen(≥95%),and those in air groups were kept in room air. AQP5 protein expression in lung tissue was detected by Western blot. AQP5 mRNA expression was de-termined by real-time PCR. Results:Lung injury developed in hyperoxia exposure groups,as evidenced by severe alveolar edema,in-flammatory cell infiltration,RBC leakage and alveolar structure destruction. The protein level of AQP5 detected by Western blot was down- regulated in H group and H+ inhibitors(H+PD/SB/SP) groups than in A group(P=0.000),but the expression of AQP5 protein was markedly higher in H+SB and H+SP groups than in H group(P=0.000),and this change was more significant in H+SB group. There was no difference in AQP5 protein expression between H group and H+PD group(P=0.737). The mRNA level of AQP5 was decreased in H group but was increased in H+SB,H+SP and H groups compared with that of A group(P=0.000). How-ever,there was no change in AQP5 mRNA level between H+PD and H group(P=0.796). There was no significant change in AQP5 protein and mRNA expression between A group and A+inhibitors(A+PD/SB/SP) groups. Conclusion:Hyperioxia decreases AQP5 protein and mRNA expression while SB203580 and SP600125 up-regulates AQP5 gene expression. Inhibition of p38 and JNK pathway blocks the effect of hyperoxia on AQP5 expression. These data show that p38 and JNK MAP kinases may involve in the down-regulation of AQP5 genes in HLI.

    • Treatment of pulmonary emphysema sheep model through bronchoscopic lung volume reduction:an experimental study

      Abstract:Objective:To explore the effectiveness and superiority of establishing airway bypass through bronchoscopic radio frequency catheter lung volume reduction in the sheep model of pulmonary emphysema. Methods:Sixteen female sheep aged 6 months were di-vided into group A and group B randomly and emphysema models were established by means of papain. Group A set up airway bypass through bronchoscopic radio frequency catheter. Both groups were given medical treatment. Forced expiratory volume in one second(FEV1.0) of every sheep before the treatment,24 h,4 weeks and 8 weeks after the treatment were measured and compared. Tho-racic CT of group A was examined regularly. All sheep were executed at 8 weeks after the treatment and were applied to do patho-logical examination. Results:①sixteen sheep were copied into pulmonary emphysema model successfully. No obvious intraoperative and postoperative abnormality was observed in group A. ②Thoracic CT of group A at 4 weeks and 8 weeks after the operation showed that:lung volume of the treatment area shrinked and atelectasis appeared.③FEV1.0 levels at different time points were significantly different(Ftime=1 352.409,P=0.000);treatment effects of two therapies were significantly different(Fgroup=5 004.278,P=0.000), and ther-apies and time were interactive(Ftime×group=1 325.058,P=0.000). ④Pathological examination showed that:in group A,alveolar at-electatic area was seen in treatment area and emphysema in non treatment area;granulocyte and lymphocyte infiltrated in alveolar atelectasis. Alveolar tissue emphysema was seen in group B. Conclusion:Establishing the airway bypass through bronchoscopic ra-dio frequency catheter lung volume reduction is effective and is superior to medical treatment in the sheep model of pulmonary em-physema.

    • Effect of androgen depletion on expression of the alveolar ENaC-?琢 in male rats with oleic acid -induced acute lung injury/acute respiratory distress syndrome

      Abstract:Objective:To study the effect of androgen depletion on expression of alveolar epithelial sodium channel alpha-subunit(ENaC-?琢)in male rats with oleic acid (OA)-induced acute lung injury/acute respiratory distress syndrome(ALI/ARDS) by the way of castrating both gonads. Methods:Twenty adult Sprague-Dawley(SD) male rats were randomly divided into control group,OA model group,castrated model group and sham-castrated model group according to the random digits table with 5 rats in each group. The model was copied by the injection of OA(0.15 ml/kg) through tail vein,and the castrated model group and sham-castrated model group were castrated 2 weeks before exposure to OA. After 6 h exposure to OA,the chest was opened to retrieve lung issues,the wet/dry(W/D) ratio was measured and the expression of mRNA and protein of ENaC-?琢 was determined by RT-PCR and Western blot respectively. Results:①The pathological change of lung tissue was distinctive and W/D ratio was significantly increased in all model groups com-pared with those in control group(OA model group vs. control group,P=0.000;sham-castrated model group vs. control group,P=0.000;castrated model group vs. control group,P=0.000). The expression of mRNA and protein of ENaC-?琢 was significantly lower in model groups than those in normal group(ENaC-?琢 mRNA:OA model group vs. control group,P=0.000;sham-castrated model group vs. control group,P=0.000;castrated model group vs. control group,P=0.001;ENaC-?琢 protein:OA model group vs. control group,P=0.000;sham-castrated model group vs. control group,P=0.000;castrated model group vs.control group,P=0.045). ②The pathological change of lung tissue was attenuated and W/D ratio was significantly decreased in castrated model group compared with those in OA model group(P=0.000). The expression of mRNA and protein of ENaC-?琢 in castrated model group was significantly up-regulated compared with that in OA model group(ENaC-?琢 mRNA:castrated model group vs. OA model group,P=0.002;ENaC-?琢 protein:castrated model group vs. OA model group,P=0.030). Conclusion:Androgen may restrain the expression of ENaC-?琢 in lung issues in male rats with ALI/ARDS.

    • Effects of fluticasone propionate/salmeterol and tiotropium on arterial elastic function of patients with moderate to severe chronic obstruction pulmonary disease

      Abstract:Objective:To explore the effects of fluticasone propionate/salmsalmeterol(FSC)and tiotropium on arterial stiffness in pa-tients with stable chronic obstruction pulmonary disease(COPD). Methods:All 136 patients with stable COPD were randomly divided into three groups:FSC group(group F,n=45),tiotropium group(group T,n=45) and blank control group(group B,n=46). Patients in group B received the conventional therapy while those in group F FSC per day for 12 weeks and those in group T inhaled tiotropium per day for 12 weeks. Changes in brachial-angle pulse wave velocity(baPWV),large and small arterial compliance(C1 and C2),augementation index(AI),high sensitive C-reactive protein(hsCRP) were observed. Results:①Multiple stepwise regression analysis demonstrated that body mass index(P=0.000),course of disease(P=0.000),hsCRP(P=0.000) and smoking index(P=0.002) were pos-itively associated with baPWV while the forced expiratory volumein one second(FEV1)(P=0.000) was negatively associated with baPWV. ②At 12 weeks after the treatment,hsCRP and C1,C2 were improved compared with those before the treatment(P<0.01),more obviously in group F(P<0.05). Significant differences presented in the above indexes in comparison between group F,group T and group B after the treatment(P<0.05). The arterial elastic paramenters of group B and AI of group F and group T were not sig-nificantly improved(P >0.05). ③baPWV of group F and group T was decreased along with the time,more obvious in group F and most significantly in group c(baPWV>21.21 m/s) among subgroups of group F. No decrease of baPWV was observed in group B. Conclusion:Fluticasone propionate/salmsalmeterol had broader impacts on arterial stiffness of patients with COPD compared with tiotropium.

    • Infection rate of pneumocystis in immunocompromised patients without HIV infection in Chongqing

      Abstract:Objective:To investigate the infection rate of pneumocystis carinii(PC) in human immunodeficiency virus(HIV) negative immunocompromised patients in Chongqing in order to provide biological references for its early diagnosis and treatment. Methods:Sputum specimens were collected from 71 HIV-negative immunocompromised patients in Chongqing and were detected by Gomori’s methenamine silver stained(GMS) and internal transcribed spacer(ITS)-nested PCR,respectively. Results:PC was detected out in two cases out of 71 cases by GMS and PC was detected out in 25 cases by nested PCR,the difference between the two methods were significant(?字2=21.04,P=0.000) .There was no significant difference in infection rate between difference genders( ?字2=0.375,P=0.540) and different ages(?字2=0.777,P=0.678). The infection rates of patients with cancer,heamatolagic disorder,kidney transplantation,liver transplantation and connective tissue disease were 33.33%,26.67%,40.00%,42.86% and 39.13% respectively. There was no signifi-cant difference in infection rates between the different case groups(P=0.897). The infection rate was significantly higher than that of abroad report and was the same as that of domestic report in 2009. Conclusion:The infection rate of PC of HIV-negative immuno-compromised patients in Chongqing is high. Clinician should enhance the alertness of patients with PC.

      Abstract:Objective:To determine the accuracy and the role of bronchoscopic radioisotope injection technique for detecting the sen-tinel lymph node(SLN) in patients with non-small cell lung cancer. Methods:Twenty patients with resectable non-small cell lung cancer(NSCLC) were included. A dosage of 18.5-55.5 MBq 99Tcm-labeled sulfur colloid was endoscopically administrated for pa-tients before anesthesia. The radioactivity of primary tumor and background was gathered after thoracotomy with a hand-hold gamma probe. Patients underwent routine lung resection and mediastinal lymphadenectomy. Radiolabeled nodes were also examined in vitro separately. Routine pathological examination was implemented and immunohistochemistry(ICH) was carried out for micrometastases. Results:Identification of SLNs was in 17 patients. In 12 patients,a unique SLN was identified and in 3 patients two SLNs were de-tected,whereas in two patients,nodes from three different stations could be classified as SLNs. The sensitivity and specificity were 53%(9/17) and 100%(3/3). Conclusion:Using bronchoscopic radioisotope injection technique to identify sentinel lymph node in patients with NSCLC is reliable. It could be a physiological way to map SLN.

      Abstract:Objective:To summarize the clinical experiences of treating central lung cancer with sleeve-lobectomy resection. Methods:From January 2002 to December 2012,41 cases of sleeve-lobectomy resection were finished,in which there were 15 cases of bronchus sleeve lobectomy,17 cases of bronchus sleeve lobectomy combined with the pulmonary arterioplasty and 9 cases of double-sleeve pulmonary lobectomy with bronchus pulmonary artery. Clinical experiences were summarized and retrospectively analyzed. Results:After the surgery,one patient with bronchopleural fistula died,one got hemoptysis,five got pulmonary infection and five got pulmonary atelectasis. Conclusion:Sleeve-lobectomy resection is safe and effective in the treatment of central lung cancer,which can expand the operative indications and reduce the loss of lung function to a maximum degree.

      Abstract:Objective:To discuss the diagnosis and treatment of pulmonary artery sling(PAS). Methods:Sixteen patients who diagnosed as PAS and received surgery in department of cardiothoracic surgery in the Children’s Hospital of Chongqing Medical University Between December 2009 and March 2013 were enrolled. Results:Sixteen patients with PAS underwent the surgery and all cases had a history of recurrent cough and wheezing. The age of patients ranged from 2 months to 3.5 years and body weight ranged from 4.5 kg to 15 kg. According to Cotton standard,14 patients in phase Ⅰ only received re-implantation of the left pulmonary artery(LPA) and 13 recovered;2 patients with trachea stenosis in phase Ⅲ received tracheoplasty at the same time but all died. Conclusion:For most PAS patients with trachea stenosis in phase Ⅰ,only re-implantation of the LPA may result in good results. For the PAS with tra-chea stenosis in phase Ⅱ and Ⅲ,early tracheoplasty for trachea stenosis remains to be discussed further.

      Abstract:Objective:To summarize the experiences of diagnosis and treatment of neonatal congenital diaphragmatic hernia(CDH) in our hospital in thirteen years. Methods:The clinical data and preoperative and postoperative blood gas data of 85 CDH cases from January 2000 to October 2013 were collected and retrospectively analyzed. Results:Totally 57 male patients and 28 female patients were enrolled including 83 cases of left diaphragmatic deletion and 2 cases of right diaphragmatic deletion. Main clinical symptoms were vomit,breathlessness and cyanosis. Percussion was performed and bowel sound was heard occasionally in sick side. Cystic bubbles were observed in the thoracic cavity but were disappeared in facies diaphragmatica and were shifted in mediastinum and heart shadow in thoracic radiograph. Facies diaphragmatica was disappeared and intestinal canal was found the thoracic cavity by iodipin opacifi-cation. Diaphragm of 82 cases was repaired through the abdomen and diaphragm of 3 cases was repaired through the thoracic cavity. On 3 d after the operation,symptoms,signs,imaging studies and pH,PCO2,PO2 in arterial blood of all patients were recovered[(pH,PCO2,PO2 values were (7.29±0.05),(7.06±0.16),(9.70±0.28) kPa respectively before the operation and were (7.38±0.03),(4.87±0.15),(11.91±0.21) kPa respectively after on 3 d after the operation,P<0.05)]. Moreover,there was no dead case of neonatal congenital diaphragmatic hernia in our hospital. Conclusion:Patients should be prepared actively before the operation when CDH is comfirmed. The symptoms,signs,imaging studies and blood gas analysis of the patients can return to normal after repair of diaphrag-matic hernia.

      Abstract:Objective:To explore the application of Vidatak EZ communication board in acute exacerbation in chronic obstructive pul-monary disease(AECOPD) patients who received invasive mechanical ventilation. Methods:Totally 131 patients who admitted into respiratory ICU of our hospital from October 2011 to November 2012 were enrolled in the study. They were randomly divided into two groups:experimental group(n=65) and control group(n=66).Vidatak EZ board was used to communicate with patients in experimental group while text card was used to communicate with patients in control group. General condition of the two groups were recorded. In-cidence of ICU delirium in two groups were assessed by confusion assessment method for ICU. Mechanical ventilation duration,ICU duration and total hospital duration,treatment outcome and incidence of ICU delirium were compared between the two groups. Results:No statistical difference was noted in general condition,acute physiology and chronic health evaluationⅡ,ICU duration,total hospital duration and treatment outcome between two groups(all P >0.05). Duration of mechanical ventilation was less in experimental group than in control group with statistically differences(P<0.05). Incidence of ICU delirium was lower in experimental group than in con-trol group with statistically differences(P<0.05). Conclusion:Vidatak EZ communication board can effectively shorten the duration of mechanical ventilation and reduce the incidence of ICU delirium in AECOPD patients who received invasive mechanical ventila-tion,therefore it has good application prospect.

      Abstract:Objective:To observe the effects of neural stem cells (NSCs) in vivo on neural regeneration in skin wounds after acidic fi-broblast growth factor(aFGF) being used to induce rat bone marrow mesenchymal stem cells(BMSCs) into neural stem cells(NSCs) in vitro. Methods:SD rat BMSCs were isolated and cultured by the whole bone marrow adherent culture method,induced by aFGF to differentiate into NSCs,and was marked with DAPI. Eighty-one SD rats were made back full-thickness defect skin wounds and each group had 9 SD rats. NSCs were randomly divided into the local wound injection(group A),the tail vein injection(group B),the NSCs with plasma graft implantation(group C). BMSCs were divided into the local wound injection(group D),the tail vein injection of(group E),BMSCs with plasma graft implantation(group F),the plasma graft transplantation(group G). Tail veins were injected with normal saline(group H) and local wounds were injected with normal saline(group I). The expression of protein gene product(PGP) 9.5 in wound skin after transplantation at the 4th,5th,7th week was detected by immunofluorescence and RT-PCR in order to reflect the skin nerve regeneration. Results:There were statistically significant differences in fluorescence density value of PGP 9.5 at the 4th,5th,7th week after the wound healing based on the results of repeated data analysis of design information(F=27.03,P=0.000). There were differences in specific expression of nerve restoration after stem cell transplantation based on the results of repeated data analysis of design information(F=130.44,P=0.000). The inter-action of stem cell transplantation in terms of time and group was statistically significant(F=3.543,P=0.001). Local injection of NSCs transplantation significantly improved nerve restoration than tail vein injection of NSCs transplantation(P=0.001). Tail vein injection of NSCs transplantation significantly improved nerve restoration than NSCs plasma graft transplantation(P=0.000). Local in-jection of NSCs transplantation significantly improved nerve restoration than local injection of BMSCs transplantation(P=0.000). Local injection of BMSCs transplantation significantly improved nerve restoration than local injection of saline transplantation(P=0.000). There were statistically significant differences between local plasma graft transplantation and local injection of saline transplantation(P=0.014) an there was no difference between local injection of saline transplantation and tail vein injection transplantation(P=0.446). Detection of PGP 9.5 with RT-PCR further illustrated the above results. Conclusion:NSCs transplanted into skin wounds in the local wound injection can promote the neural regeneration of wound and it was the most effective pattern.

      Abstract:Objective:To investigate the roles of Fractalkine in triggering the migration of the mesenchymal stem cells(MSCs) in the Transwell co-culture system. Methods:MSCs were isolated and cultured through the whole bone marrow method and were amplified by the different-speed adherent method. The morphology and biological characteristics of MSCs were examined by flow cytometry. And then the recombinant adenovirus containing CX3CR1 and green fluorescent protein(GFP)(LV-CX3CR1-EGFP) or empty vector(LV-CON-EGFP) were transfected into MSCs. The expression of CX3CR1 mRNA in MSCs was examined by RT-PCR and the pro-tein expression was detected by Western blot in LV-CX3CR1-EGFP infection group,LV-CON-EGFP infection group and non-infec-tion group. The migration of MSCs was detected with Transwell co-culture system. Results:Obtained MSCs were homogeneous in cell morphology with good living condition and the cells showed positive with CD29,CD44,CD34 .There was no expression of CX3CR1 in LV-CON-EGFP group and non-infection group but CX3CR1 can be expressed in LV-CX3CR1-EGFP group. Results of Transwell assays suggested that the migration level of MCSs was higher in LV-CX3CR1-EGFP group than in other groups(P=0.000). The migrated MSCs were 13.80±2.17/HP,35.80±3.34/HP and 14.80±1.92/HP in LV-CON-EGFP group,LV-CX3CR1-EGFP group and non-infection group respectively. Conclusion:The transfection of CX3CR1 gene could improve the level of migration of MSCs by FKN in vitro.1

      Abstract:Objective:To explore the effects of all trans retinoic acid(at-RA) on rat bone marrow mesenchymal stem cells(rBMSCs) osteogenic differentiation and mechanisms. Methods:The rBMSCs were isolated and induced to differentiate into osteoblast. At-RA at concentrations of 0.1,1.0,5.0 μmol/L was added to the osteoblast induced medium. Morphology of the cultured cells was observed us-ing inverted microscope respectively. On the 7th d,histochemical staining and alkaline phosphatase detection kit were used to test the alkaline phosphatase(ALP) activity. On the 21st d,alizarin red staining was employed to detect calcium salt deposition. During osteo-genetic induction,mRNA expressions of both osteocalcin(OCN) and osteopontin(OPN),biomarkers of osteoblast and related differen-tiation regulatory genes,i.e. Runx2,osterix of the osteogenesis regulation pathways and retinoic acid receptors(RARs,?琢,?茁) of the retinoic acid signaling pathways were measured using real-time PCR. Results:On the 7th d of differentiation,the activity of ALP of os-teogenic induced rBMSCs with at-RA cells increased markedly compared with that without at-RA cells(F=107.177,P=0.000). How-ever,on the 21st d,the calcium salt deposition of at-RA treated rBMSCs significantly reduced compared with that of non-treated rBMSCs(F=57.554,P=0.000). During osteogenic induction process,OCN,OPN,Runx2,Osterix and RAR?琢 expressions of at-RA treated(5.0 μmol/L) rBMSCs decreased compared with those of non-treated rBMSCs(211.145,0.000;30.835,0.005;106.536,0.000;452.546,0.000;253.159,0.000,respectively for F and P values) on the 21st d. Meanwhile,significantly increased RARβ expression was observed(F=2 294.377,P=0.000) during the whole differentiation process. Conclusion:at-RA can inhibit osteogenic differentiation of rBMSCs. The mechanism may be through up-regulating the expression of RARβ,down-regulating the expression of RARα,and inhibiting the expressions of Runx2 and Osterix.

    • Survival of 118 patients with malignant hematological disease treated by autologous hematopoietic stem cell transplantation

      Abstract:Objective:To evaluate the clinical efficacy of autologous hematopoietic stem cell transplantation(auto-HSCT) in the treatment of malignant hematological diseases. Methods:The clinical data of 118 patients with hematological diseases who received auto-HSCT were collected. Of the 118 patients,23 were acute leukemia(AL),69 were malignant lymphoma(ML) and 26 were multiple myeloma(MM). The overall survival(OS),disease free survival(DFS),and the relapse rate were analyzed. Results:Totally 117 patients engrafted successfully,except one patient died because of infection. The average time was 9.8 d for neutrophil reconstruction(≥0.5×109/L),and 12.3 d for platelet reconstruction(≥20×109/L). Median survival time was 47.6(1-80) months. The 3-year OS and DFS were 62.3% and 54.3%,the 5-year OS and DFS were 49.0% and 42.8%,and the cumulative relapse frequency was 28.0%.The prognosis of ML was better than that of AL and MM(P<0.05). Conclusion:Auto-HSCT is effective for AL. Auto-HSCT can obviously prolong the survival time of ML and MM patients,and the MM should get individualized treatment.

      Abstract:Objective:To establish a reversed-phase high-performance liquid chromatographic method to determine the content of naringin in nanofibers. Methods:The test film,nanofiber film containing naringin,was prepared using electrospinning equipment. The determination was performed on Agilent C18 chromatographic column(4.6 mm×150 mm,5 μm). The acetonitrile-phosphoric water (pH 2.8) (23∶77) was used and the detecting wavelength was 282 nm. The column temperature was 30 ℃. The sampling inlet vol-ume was 20 μl. Results:Pure PCL nanofiber,having good specificity exerted no interfering effect on naringin determination. The low-est detection limit of naringin S/N=3 was 3.4 ng/ml. The limit of quantitation S/N=10 was 10.2 ng/ml. Good linear dependence(corre-lation coefficient r=0.999 99) was detected when the concentration of naringin was within the range of 51-1 836 μg/ml. This method had good precision and repeatability both within and between days. The average recovery of naringin was 99.51% and the relative standard deviation was 0.66%. The test solution was stable within 24 h. Conclusion:Naringin nanofiber film can be produced using electrospinning technique which is a nano film with medicine loaded for bone regenerative. This study provides an easy,accuracy,rapid way for the quantitative determination of naringin in nanofibers and solid basis for the evaluation of the optimum content of naringin in nanofibers.

      Abstract:Objective:To prepare superparamagnetic Fe3O4 chitosan nanoparticle and to explore the effects of different surfactants and crosslinkers on prepared nanoparticles. Methods:The nanoparticle was prepared by chemical co-precipitation method. The size,poly-dispersity index and zeta potential was detected by Zetasizer Nano. The magnetization intensity was detected by vibration sample magnetometer. Results:The size of the nanoparticle ranged from 15 nm to 60 nm,with an average size of 29.39 nm. Polydispersity in-dex was 0.185 and the absolute value of zeta potential of the nanoparticle was approximately 20. Magnetic induction was very well. Conclusion:The surfactants with low hydrophile lipophilic balance value can reduce the size of nanoparticle. Glutaraldehyde dosage should be controlled between 1.5 ml and 2.0 ml. Concentration of NaOH solution should be controlled between 3.0 mol/L and 4.0 mol/L.

      Abstract:Objective:To study methods of modeling of fluid-structure interaction model of complex intracranial aneurysms. Methods:The original data series of left internal carotid artery spiral giant fusiform aneurysm in a patient were got by 3D cerebral angiography. The original data series was processed to obtain a surface model of STL format of the aneurysm’s vessel segment using MIMICS 12.0.The surface model was imported into Geomagic Studio 12 and was processed to get double NURBES surfaces,which were processed to get the fluid-structure interaction model of internal carotid artery aneurysm using UGS NX 8.0. The model was imported into ANSYS Workbench 14.0 and fluid-structure interaction calculation was performed to evaluate its reliability. Results:A fluid-structure inter-action model was built rapidly and quickly,which was almost the same as actual entities of the patient and was good for fluid-struc-ture interaction analysis. Conclusion:In combination with MIMICS 12.0,Geomagic Studio 12 and UGS NX 8.0,reliable fluid-structure interaction model of complex intracranial aneurysms can be quickly built. This provides a reliable method for the numerical simulation of cerebrovascular disease.

      Abstract:Objective:To access the influence of hematuria on the performance of urine cytology(CYT) and fluorescence in situ hy-bridization(FISH) in the detection of bladder carcinoma(BC). Methods:From January 2009 to June 2013,a total of 164 patients sus-picious of BC underwent tests of CYT and FISH followed by upper urinary tract imaging and cystoscopy. Transurethral biopsy and/or resection were performed on suspicious lesions. The hematuria was determined by urine microscopy. Effect of hematuria on false pos-itive and negative rates of patients with and without BC was compared. Results:For patients with hematuria,false positive rates of CYT were 14.3% and 20.9%(χ2=0.645,P=0.422) and false negative rates of CYT were 21.4% and 39.1%(χ2=2.618,P=0.106);false positive rates of FISH were 19.0% and 20.9%(χ2=0.047,P=0.828) and false negative rates of FISH were 30.3% and 30.4%(χ2=0.002,P=0.967) all without statistical difference. Conclusion:Hematuria does not influence the performance of CYT and FISH.

      Abstract:Objective:To investigate the accuracy of digital models of full crowns which were constructed by three types of dental gypsum materials in Cerec Inlab MC XL system. Methods:A upper first right molar in the standard model was prepared,and eight scanned models made of superhard gypsum,Esthetic-base gypsum,and CAM-base were duplicated respectively. Eight digital models in each group were digitally imaged by scanning in InEos unit with Rotational method(superhard gypsum was scanned after spraying powder). The distance parameters of each digital model were measured by inlab 3D software(version 3.65) and compared with mate-rial object. Finally,the experimental data were analyzed by SPSS 19.0 statistical software. Results:The quality of digital imaging was better in superhard gypsum group than in Esthetic-base and CAM-base group. The observed values of Esthetic-base group,CAM-base group and the real group had no statistical difference in terms of buccal-lingual(P=0.199),and mesial-distal directions(P=0.087),but the observed values of the superhard gypsum group and real group were statistically different(P=0.000,P=0.003). In addition,the deviation values of superhard gypsum group were greater in mesial-distal directions than in buccal-lingual directions,with statistical-ly significant differences(F=7.310,P=0.000). The overall offset of superhard gypsum group,Esthetic-base group and CAM-base group were (0.049±0.036),(0.016±0.050) mm and (0.017±0.052) mm,respectively. Conclusion:Under the condition of this ex-periment,the esthetic-base and CAM-base have high accuracy,but superhard gypsum after spraying powder is not recommended due to its inaccuracy.

    • Construction of rabbit abdominal hernia model and its criteria

      Abstract:Objective:To explore the feasibility of establishing a stable animal model of abdominal hernia and the criteria to determine its success through animal studies. Methods:The 40 male rabbits were randomly divided into A,B,C and D groups with 10 rabbits in each group. A group was taken as control group and the remaining three groups were taken as experimental groups. Full structure of abdominal wall(muscle and peritoneum)of rabbits in experimental group was cut by sizes of 2 cm×2 cm,3 cm×3 cm,4 cm×4 cm,respectively. Protruding abdominal mass,body weight and mortality of rabbits were observed one month later. Results:There was no significant difference in body weight in groups A,B,C before and after the surgery(P=0.504,0.465,0.090). Body weight was decreased in group D after the surgery compared with that before the surgery(P=0.000). No case of abdominal mass and no death case in group A,two cases of abdominal mass and one death case in group B,six cases of abdominal mass and one death case in group C,six cases of abdominal mass and three death cases in group D were observed. Conclusion:Removal of abdominal tissues by size of 3 cm×3 cm can build a stable animal model of abdominal hernia and propose a method to build hernia model and criteria,paving the way for the research of the hernia in the future.

      Abstract:Objective:To investigate the peripheral blood of von Willebrand factor-cleaving protease(vWF-cp) levels in patients with coronary heart disease(CHD) and its correlation with global registry of acute coronary events(GRACE) score then to assess the risk of CHD with the combination of them. Methods:Totally 145 patients who underwent coronary angiography were selected as research subjects. They were divided into three groups by coronary angiography:49 patients with non-coronary heart disease as control group;47 patients with stable angina(SAP) as SAP group;49 patients with acute coronary syndrome(ACS) as ACS group. All patients were scored by GRACE system and then layered. Meanwhile,degree of coronary artery stenosis was assessed by Gensini scoring system and vWF-cp level was measured by ELISA. Results:vWF-cp levels in control group were strikingly higher than those in ACS group(P=0.000) and also higher than those in SAP group(P=0.000);vWF-cp levels in ACS group were obviously decreased compared with those in SAP group(P=0.023). GRACE scores in ACS and SAP groups were increased compared with those in control group(P=0.000)(P=0.008);GRACE score in ACS group was increased compared with that in SAP group(P=0.033). There was no significant difference in vWF-cp level among CHD patients with different Gensini scores(r=0.060,P=0.616),but GRACE score was negatively correlated with vWF-cp levels(r=-0.672,P=0.000). Conclusion:Combination of GRACE score and vWF-cp levels could predict risk and progno-sis in patients with CHD accurately and efficiently.

      Abstract:Objective:To explore the correlation of the glucokinase regulatory protein(GCKR) gene rs3817588 and rs780094 single nucleotide polymorphisms(SNPs) with polycystic ovary syndrome(PCOS) and its related metabolic indices in Chongqing Han popu-lation. Methods:Totally 147 patients with PCOS and 156 normal controls were recruited and genotyped by TaqMan SNP genotyp-ing assay;Hyperinsulinemic-euglycemic clamp was performed on all patients and 20 controls. Results:Nither statistical differences in genotype and allele frequencies of rs3817588(OR=0.929,95%CI=0.541 to 1.594, ?字2=0.72,P=0.79;OR=0.995,95%CI=0.759 to 1.304,?字2=0.00,P=0.97,respectively),nor statistical differences,in genotype and allele frequencies of rs780094(OR=1.427,95%CI=0.974 to 1.598, ?字2=2.74,P=0.10;OR=0.880,95%CI=0.700 to 1.107, ?字2=1.20,P=0.27,respectively) was found between the two groups. Conclusion:GCKR gene rs3817588 and rs780094 polymorphisms have no association with PCOS or its related metabolic factors in Han population in Chongqing.

    • Risk factors of frequent atrial premature beats and atrial tachycardia

      Abstract:Objective:To explore risk factors of frequent atrial premature beats(FAPBs) and atrial tachycardia(AT). Methods:Case-control study was conduced to check Holter reports of patients in the First Affiliated Hospital of Chongqing Medical University during July 2011 to June 2012. Totally 478 patients were enrolled including 230 patients with FAPBs and 248 patients with sporadic atrial premature beats(SAPBs) or putting another way,147 patients with AT and 61 patients with atrial premature beats(APBs). Dif-ferences in constituent ratio among different factors between FAPBs and SAPBs were compared. The above statistically sig-nificant factors of FAPBs relative to SAPBs were analyzed by logistic regression and risk factors of AT relative to APBs were analyzed by the same way. Results:Age(OR=1.018,P=0.049),history of hypertension(OR=1.653,P=0.035) and heart function(OR=1.421,P=0.006) were potential risk factors of FAPBs relative to SAPBs. Age(OR=1.060,P=0.000) and valvular heart disease(OR=1.584,P=0.012) may be risk factors of AT relative to APBs. Conclusion:Early prevention and treatment of hypertension and valve lesions is likely to reduce the incidence of FAPBs and AT. Elderly FAPBs and AT patients needn’t be intervened actively.

    • Comparative study of effects of combined spinal-epidural anesthesia and epidural anesthesia on haemodynamics in the puerpera with cesarean delivery

      Abstract:Objective:To research and improve the anesthesia management of cesarean delivery and to provide references for clinical to ensure the maternal and child safety. Methods:Eighty puerpera with cesarean delivery were randomly assigned to receive com-bined spinal-epidural anesthesia(CSEA group) or epidural anesthesia(EA group) with 40 cases in each group. Homodynamic changes,anesthetic effect,neonatal Apgar score,complications and adverse reactions were measured in the two groups. Results:Heart rate(HR) of puerpera was higher in CSEA group than in EA group at T4(10 min after child birth)and T5(operation finish)(P=0.023,0.022). Systolic blood pressure(SBP) and diastolic blood pressure(DBP) were higher in CSEA group than in EA group at T3 (child birth)(P=0.000,0.000). Mean arterial pressure(MAP) was higher in CSEA group than in EA group at T2(satisfied anesthesia-before disinfection)and T5(operation finish)(P=0.000,0.003). There was no differences in cardiac output(CO),stroke volume(SV),systemic vascular resistance(SVR) and thoracic fluid content(TFC) between two groups at different time points(all P >0.05).There were differences in HR,SBP,DBP,MAP,CO,SV and SVR within each group at different time points(P<0.05). Anesthetic effect of CSEA group was better than that of EA group(P=0.000). There were differences in neonatal Apgar score,complications and adverse reactions between two groups(P >0.05). Conclusion:EA and CSEA are both suitable anesthesia for the puerpera with cesarean de-livery. However,CSEA is more recommended for its faster onset,better anesthetic effect and little effect on maternal hemodynamics.

    • Effect of carbon dioxide pneumoperitoneum on neuromuscular blockade of rocuronium in patients undergoing laparoscopic surgery

      Abstract:Objective:To evaluate the effect of carbon dioxide pneumoperitoneum on neuromuscular blockade of rocuronium in patients undergoing laparoscopic surgery. Methods:Sixty patients sche-duled for either laparoscopic or open hysterectomy procedures under general anesthesia were enrolled,30 patients in each group. The same anesthetic protocol was performed to all pa-tients. The responses of adductor pollicis to train of four(TOF) stimulation of ulnar nerve were monitored. After intubation,each patient received rocuronium by continuous infusion with initial rate of 6 μg/(kg?min) to maintain the ratio of T1 and Tc(T1/Tc) at the level between 0% and 10%. The time interval from the end of rocuronium infusion to 25% recovery of T1/Tc(T1/Tc 25%),recovery in-dex(T1/Tc from 25% to 75%),the duration of TOF from 0% to 90%(TOF90%),dosages of rocuronium administered during anesthesia maintenance were recorded. Results:The demographic data including age,weight,height,body mass index(BMI),surgical time,dura-tion of rocuronium infusion were comparable between the groups. T1/Tc 25%,recovery index and TOF90% were significantly longer in patients underwent laparoscopic hysterectomy than in those underwent open hysterectomy(P<0.01). The amount of rocuronium used anesthesia maintenance was less in laparoscopic group than in open group(P=0.001). Conclusion:Carbon dioxide pneumoperitoneum appears to slow down the metabolism of recoronium,which may result in prolonged duration of action,as well as delayed recovery after muscle relaxant administration in laparoscopic surgery.

    • Early non-surgery treatment for ectropion after blepharoplasty

      Abstract:Objective:To investigate a non-surgical method for treating ectropion caused by lower-blepharoplasty at the early stage. Methods:Firstly,the elastic tape was used to tract and to fix the lower eyelid skin after blepharoplsty as early as possible. Secondly,if the ectropion was not corrected entirely after 14 d,triamcinolone acetonide(10 mg/ml) should be locally injected into the lower-eye-lid combined with using elastic tape. Results:Totally 15 cases(20 eyes) with mild to moderate ectropion underwent the treatment and all eyes obtained good outcome without ectropion. Conclusion:Mild to moderate ectropion caused by lower-blepharoplasty can be corrected by non-surgical treatment at the early stage. Elastic traction combined with triamcinolone acetonide is a simle,safe,effective method.

    • Brachiocephalic artery conserved Sun’s procedure(with a report of 4 cases)

      Abstract:Objective:To summarize the experiences of brachiocephalic artery conserved Sun’s procedures for the patients with bra-chiocephalic artery uninvolved complicated Stanford type A aortic dissection. Methods:From February 2013 to November 2013,4 pa-tients of innominate artery uninvolved complicated Stanford A aortic dissection were operated in our hospital. They were all males,with an average age of (45±16.02) years old. Two patients were operated in emergence while two others not. Right axillary artery cannulation was routinely used for cardiopulmonary bypass and selected cerebral perfusion was utilized when deep hypothermic circu-latory arrest. Replacement of ascending aorta was carried out for three patients,while replacement of noncoronary sinus and ascending aorta for one. Results:Cardiopulmonary bypass time was (220±58.21) min,the aortic occlusion time was (109.25±52.87) min,and selected cerebral perfusion time when deep hypothermic circulatory arrest was (53.75±29.24) min. The drainage in 24 h was (337.5±202.22) ml after operation. No coma and paraplegia was found in this group. No patient was re-operated. One patient suffered from noninvasive mechanical ventilation because of respiratory insufficiency. All patients were discharged home. CT exams before discharge confirmed a good result of surgery. All patients were follow-up for 1-10 months. They recovered well and no death occurred. Conclusion:Brachiocephalic artery conserved Sun’s procedures is good and safe for the patients with brachiocephalic artery uninvolved complicated Stanford A aortic dissection.
