    • Expression and clinical significance of A20 and NLRP3 genes in juvenile idiopathic arthritis

      Abstract:Objective To analyze the expression of A20 and NLRP3 genes in healthy controls(HCs) and those with juvenile idiopathic arthritis(JIA),so as to investigate its role in predicting the efficacy of biological agents in JIA patients.Methods According to the 2001 International League of Associations for Rheumatology(ILAR) diagnostic criteria for juvenile idiopathic arthritis,33 HCs and 68 children newly diagnosed with JIA were enrolled in the study,and their peripheral blood mononuclear cells(PBMCs) were collected. Quantitative real-time PCR(qRT-PCR) was used to detect the mRNA expression of A20 and NLRP3 genes. Inactive disease was regarded as clinical remission(CR) according to the Juvenile Idiopathic Arthritis Disease Activity Score proposed by the American College of Rheumatology(ACR) in 2014. The differences in the mRNA expression level of A20 and NLRP3 genes were compared between the newly diagnosed children and HCs. According to different treatment options,JIA patients were divided into IL-6 inhibitors group,TNF-α inhibitors group and non-biological agents group. The correlations between the expression of these two genes and the efficacy after six months of treatment were analyzed.Results The expression level of A20 in JIA group was significantly lower than that in HCs(P=0.039). The expression level of A20 in systemic JIA(sJIA) group was lower than that in non-systemic JIA(non-sJIA) group(P=0.005). There was no significant difference in the expression of NLRP3 gene between the two groups(P>0.05). There was no significant correlation between the expression of A20 and NLRP3 gene with the therapeutic efficacy by the binary logistic regression analysis(P>0.05). After six months’ treatment,NLRP3 expression in the CR group was lower than that of the clinical non-remission group(P<0.05),and there was no significant difference in A20 gene expression between CR group and non-remission group(P>0.05).Conclusion The expression of A20 is significantly decreased in children with JIA. The expression of A20 in sJIA is lower than that in non-sJIA. Low expression of NLRP3 gene in children with JIA may have a higher treatment response rate and better prognosis. No correlation has been found between the expression of A20 and NLRP3 genes and the therapeutic efficacy of JIA yet.

    • Inhibitory effect of dihydroartemisinin combined with cisplatin on the proliferation of bladder cancer cells

      Abstract:Objective To investigate the effects of dihydroartemisinin(DHA) combined with cisplatin(DDP) on inhibiting the proliferation,invasion and metastasis of bladder cancer cells.Methods The bladder cancer cells T24/5637 were used as research subjects,and half maximal inhibitory concentration(IC50) of DDP and DHA were respectively detected by CCK-8. Flow cytometry was used to detect the effects of DHA and DDP alone or in combination on the cycle and apoptosis of the two cell lines. The effects of DHA and DDP alone or in combination on the migration and invasion of bladder cancer cells were detected by scratch test and Transwell test.Results The IC50 of DDP against 5637 cells and T24 cells were 17.10 μg/mL and 15.28 μg/mL,respectively. The IC50 of DHA was 24.08 μg/mL for 5637 cells and 32.27 μg/mL for T24 cells. The blocking effect of DHA/DDP alone on G0/G1 phase was weaker than that of the combination group(P<0.05),and there was no statistical significance in the inhibition effect of DHA/DDP alone and in combination on G2/M phase. The apoptosis rate of T24 and 5637 cells induced by DHA combined with DDP was significantly higher than that of DHA/DDP alone(P<0.05),and the apoptosis rate induced by DDP was significantly higher than that of DHA(P<0.05). At 24 h,the percentage of migration area in DHA group was larger than that in DDP group(P<0.05) and DDP+DHA group(P<0.05),and the percentage of migration area in DDP group was larger than that in DDP+DHA group(P<0.05). After 24 h,the inhibitory rate of DHA group was significantly lower than that of DDP group(P<0.05) and DDP+DHA group(P<0.05),and the inhibitory rate of DDP group was significantly lower than that of DDP+DHA group(P<0.05).Conclusion DDP+DHA could enhance the inhibitory effect of DDP on the proliferation of bladder cancer cells,and DDP+DHA could further enhance the inhibitory effect of DDP on the migration and invasion of bladder cancer cells.

      Abstract:Objective To investigate the role of Bcl-2 associated athanogene 4(BAG4) in self-renewal capacity of gastric cancer stem cells(GCSCs).Methods GCSCs were isolated/enriched using sphere forming culture system, and their biological characteristics were identified. BAG4-knockdown MGC803 gastric cancer cell lines were constructed by lentivirus interference technique. Colony and tumor sphere formation assay were used to estimate the effect of BAG4-knockdown on self-renewal capacity of gastric cancer in vitro.Results We successfully isolated/enriched sphere cells(SC) using sphere forming culture system. MGC803-SC showed higher expression level of stem-related genes Sox2, Oct4 and Bmi1 than normal adherent cells, and showed stronger clone-formation ability(198±17 vs. 92±11,P<0.001). After induction of differentiation, the expression of CD44 decreased,and showed stronger invasion ability in vitro(185±18 vs. 84±11,P<0.01). The above experimental results confirmed that MGC803-SC had GCSCs properties. The expression level of BAG4 in GCSCs was significantly higher than that in normal adherent gastric cancer cells.Conclusion Silencing BAG4 gene can effectively inhibit the self-renewal ability of GCSCs,suggesting that BAG4 might be a promising target for gastric cancer therapy.

      Abstract:Objective To explore the protective effect and corresponding mechanisms of 3-methyladenine(3-MA) on liver injury induced by sleep disturbance.Methods Thirty SD mice were randomly divided into three groups,control group, modeling group,and 3-MA group, with 10 mice in each group. After the end of exposure, the weight changes in mice were measured and compared among the groups before and after the experiments. After the mice were sacrificed, the following experiments were made. ①The level changes of alanine aminotransferase(ALT) and aspartate aminotransferase(AST) were detected by ELISA method. ②HE staining was conducted to observe the histomorphological changes of the liver. ③Autophagosomes were analyzed using transmission electron microscope. ④TUNEL method was used to evaluate the apoptosis level of hepatocytes in each group and calculate the apoptosis index. ⑤RT-PCR and Western blot methods were used to detect the mRNA and expression levels of autophagy-related proteins,such as microtubule-associated protein light-chain-3 (LC3-Ⅱ),Beclin1,glucose-regulated protein 78(GRP78),and Caspase-3.Results The animal experiments showed that sleep disturbance could induce the liver damage,and which was accompanied with the increase of autophagy and endoplasmic reticulum stress(ERS). Compared with the control group,the body weight increment of the model group decreased significantly,while the morphological structure of hepatocytes remarkably changed. Meantime,the levels of ALT and AST,apoptosis index,autophagosomes and the expression of related marker proteins increased respectively(P<0.05). In contrast with the model group,the morphological structure,serum ALT and AST,apoptosis index,autophagosomes and the expression of related marker proteins were improved(P<0.05).Conclusion Sleep disturbance exposure can induce liver damage in SD rats,and then further activate autophagy and ERS signaling pathways by up-regulating the protein expression and mRNA level of LC3-Ⅱ,Beclin1 and GRP78. 3-MA can antagonize autophagy and ERS signaling pathways to protect hepatocytes and resist apoptosis.

    • Evaluation value of ultrasound in superficial temporal artery-middle cerebral artery anastomosis in treating moyamoya disease

      Abstract:Objective To conduct the preoperative and postoperative evaluation of moyamoya disease(MMD)patients undergoing superficial temporal artery(STA)-middle cerebral artery(MCA)anastomosis using ultrasound technique,and to investigate the clinical value of ultrasound in early prediction of surgical success rate.Methods A total of 28 patients who underwent STA-MCA anastomosis in The Second Affiliated Hospital of Air Force Medical University from July 2018 to September 2020 were selected. The internal diameters and hemodynamic parameters of main branch,top branch and frontal branch of STA trunk,the distal and proximal MCA and the posterior cerebral artery(PCA) were detected 1-3 days before and 4-7 days after surgery. The internal diameter and hemodynamic parameters of bridging vessels were detected after surgery. Hemodynamic parameters were measured including systolic peak flow velocity(Vs),diastolic end flow velocity(Vd),mean flow velocity(Vm),pulse index(PI) and resistance index(RI). All patients were divided into two groups according to the results of 6 months to 1 year digital saubtraction angiograph(DSA)examination after surgery,and the differences between the two groups were statistically analyzed.Results All patients underwent DSA examination within 6 months to 1 year after surgery,which confirmed that 86%(24/28)of the anastomosis vessels were patency,and the operation was successful. Compared with preoperation,92%(22/24)of patients in the patency group showed the diameter of STA and bridge vascular was significantly widened,the VsVm and Vd were significantly increased,PI value was significantly decreased after surgery,“intracranial arterialization” change appeared in spectrum,and reverse flow occurred in MCA distal segment(all P<0.05). There were no statistically significant differences in the proximal segment of MCA and PCA blood flow parameters before and after surgery. The diameter of STA and bridge vascular in the patency group was significantly wider than that in the non-patency group(P<0.05). The VsVm and Vd values of STA and bridging vascular in the patency group were higher than those in non-patency group,and the PI value was lower than that in the non-patency group,with significant differences between the two groups(P<0.05). ROC curve were showed that the change rate of STA diameter after surgery was greater than or equal to 37%,and the STA-PI value was less than or equal to 1.26,which were the best cut-off values to predict the success of surgery.Conclusion When MMD patients are undergoing STA-MCA anastomosis surgery,ultrasound technology can beused to detect the internal diameters and blood flow parameters of STA before surgery,and the internal diameter change rate and the PI of STA and bridge vessels after surgery,which has potential clinical application value for early prediction of whether the anastomotic vessels are patency and whether the operation is successful.

    • The comparison of enrichment effects of different screening schemes for fetal nucleated red blood cells from maternal blood by magnetic-activated cell sorting

      Abstract:Objective To isolate fetal nucleated red blood cells(FNRBC)from maternal umbilical cord blood through density gradient centrifugation(DGC)and magnetic-activated cell sorting(MACS) by four different screening schemes,and to evaluate the screening effect of different methods.Methods Ten samples of umbilical cord blood(12 mL/case)were collected. Each sample was divided into 4 groups:Group 1:DGC; Group 2:DGC+MACS negative screening(CD45-);Group 3:DGC+MACS positive screening(CD71+);Group 4:DGC+MACS negative/positive screening(CD45-/CD71+). After screening,the number of cells was counted,and the t-test was carried out to evaluate the enrichment efficiency of each method. K-B staining was used to identify FNRBC count,and then the number and the proportion of FNRBC were calculated.Results ①Compared with the other three groups,DGC method had the largest total cell number(0.95×107/mL),the largest FNRBC number(39.87/mL) and the lowest proportion of FNRBC(0.004‰),with statistically significant differences(P=0.000);②DGC+MACS negative / positive screening(CD45-/CD71+) method got the least total cells number(0.81×106/mL)and the least FNRBC number(7.47/mL),with statistically significant differences(P=0.000);③In the DGC+MACS single antibody screening method,the total number of cells(1.99×106 /mL)obtained by DGC+MACS positive screening(CD71+) was lower than that by DGC+MACS negative screening(CD45-)(2.56×106/mL). The difference was statistically significant(P=0.000). However,there was no significant difference in the number of FNRBC between the two methods(P=0.910);④There was no significant difference in the proportion of FNRBC between DGC+MACS negative screening(CD45-),DGC+MACS positive screening(CD71+) and DGC+MACS negative/positive screening(CD45-/CD71+)(P=0.030,0.050,0.210).Conclusion The number of FNRBC obtained by DGC is the largest. DGC+MACS negative/positive screening(CD45-/CD71+)has the strongest ability to remove mixed cells. The effect of enriching FNRBC by DGC+MACS positive screening(CD71+) is the best without reducing the number of target cells.

      Abstract:Objective To explore whether the massive gray scale change(MGSC) can benefit far-field adverse effects during focused ultrasound ablation(FUA) in the treatment of benign uterine diseases.Methods In a single-center retrospective clinical observation study,40 patients with benign uterine diseases were treated with FUA in The Second Affiliated Hospital of Chongqing Medical University. Before and after the occurrence of MGSC,the incidence of far-field adverse effects such as sacrococcygeal or hip pain,radiation pain,perineal pain,anal distension pain and the levels of adverse effects were compared under the same energy level(the lowest intolerant energy).Results Under the same energy level(the lowest intolerant energy),the incidence of sacrococcygeal or hip pain,radiation pain,perineal pain,anal distension pain and the levels of adverse effects immediately after the occurrence of MGSC were significantly lower than those before the occurrence(P<0.05).Conclusion The MGSC can reduce far-field adverse effects during FUA in the treatment of benign uterine diseases.

    • Evaluation of postoperative radiotherapy in the treatment of supraglottic laryngeal squamous cell carcinoma

      Abstract:Objective To evaluate the role of postoperative radiotherapy(PORT) in supraglottic laryngeal squamous cell carcinoma.Methods The clinical data of 831 patients with supraglottic laryngeal squamous cell carcinoma selected from the SEER(Surveillance,Epidemiology,and End Results)database who received surgical treatment in 2004 to 2015 were retrospectively analyzed. Propensity score matching(PSM) was used to balance the covariate bias between the PORT(-) and PORT(+) groups. The survival curves were drawn by Kaplan-Meier method,and log-rank test was used to check survival difference of two groups and make subgroup analysis. And the overall survival(OS) and cancer specific survival(CSS) were compared between these two groups to analyze the value of PORT in the treatment of patients with supraglottic laryngeal squamous cell carcinoma.Results There was no significant difference in OS between the PORT(-) and PORT(+) groups before the PSM,while the CSS of the PORT(+) group was lower than that of the PORT(-) group(P=0.022). The subgroup analysis of the number of positive lymph nodes showed that there was no significant difference in OS and CSS of patients with 0 or 1 positive lymph node. However,the patients with positive lymph nodes greater than 1 in PORT(+)group had significantly higher 5-year OS and CSS rate than those in the PORT(-)group(OS:23.1% vs. 38.1%,P<0.001;CSS: 34.8% vs. 45.3%,P=0.004). After PSM,the OS and CSS rate in the PORT(+) group were both better than those of the PORT(-) group(OS: 55.1% vs. 41.9%,P=0.004;CSS:67.1% vs. 56.1%,P=0.042). When analyzed by subgroup,patients with histological Ⅰ and Ⅱ stage,AJCC T4 stage,AJCC N2b stage or above,count of positive lymph nodes >1 and total laryngectomy had survival benefit for the PORT(+) group,with significant differences. And patients with lymph node ratio <20% and tumor size <30 mm without chemotherapy only had OS benefit associated with PORT.Conclusion The use of PORT can improve the OS and CSS rate in supraglottic laryngeal squamous cell carcinoma,and the related beneficiary groups are patients with AJCC T4 stage,AJCC N2b stage or above,and the count of positive lymph nodes >1,which needs to be further evaluated for the role of PORT in patients with adverse pathological features in other similar studies.

    • Predictive value of HPV E6/E7 mRNA for the outcome of cervical intraepithelial neoplasia grade 1

      Abstract:Objective To evaluate the application value of human papilloma virus(HPV) E6/E7 mRNA in predicting the outcome of cervical intraepithelial neoplasia grade Ⅰ(CIN Ⅰ).Methods From October 2017 to March 2020, a total of 208 patients admitted to the Outpatient Department of Chongqing Health Center for Women and Children with abnormalities in one of the results of thinprep cytology test(TCT), HPV DNA and HPV E6/E7 mRNA tests and then diagnosed as CIN Ⅰby colposcopy were randomly selected. A year later, these patients were retested of TCT, HPV DNA and HPV E6/E7 mRNA tests. Cervical biopsy was performed through colposcopy again for one abnormal result. Patients were followed up for regression,persistence, or progression of the lesion. It was in this way that we tried to analyze the predictive value of HPV E6/E7 mRNA for the risk of progression of CIN Ⅰ.Results ①Among the 208 patients with CIN Ⅰ,17 patients progressed to CIN Ⅱ or CINI Ⅲ, with progression rate of 8.2%. The positive rate of TCT was 58.8%,and the positive rate of both HPV DNA and HPV E6/E7 mRNA was 100% in patients with progression. ②The hazard ratio(HR) value of TCT,HPV DNA,and HPV E6/E7 mRNA in the Cox regression analysis was 9.184, 158.675,and 293.650, respectively. The HR value of HPV E6/E7 mRNA was the highest,indicating that HPV E6/E7 mRNA had the greatest predictive value for CIN Ⅰ progression. ③HPV DNA and HPV E6/E7 mRNA had the highest sensitivity. TCT had the highest specificity,followed by the HPV E6/E7 mRNA. HPV DNA had poor specificity. Thus,HPV E6/E7 mRNA had both the highest sensitivity and high specificity. ④The area under the curve(AUC) value of TCT,HPV DNA,and HPV E6/E7 mRNA in the ROC curve was 0.633,0.427,and 0.772,respectively. The AUC value of HPV E6/E7 mRNA was the highest,indicating that HPV E6/E7 mRNA had the greatest diagnostic value for CIN Ⅰ progression. The AUC value of HPV DNA combined with HPV E6/E7 mRNA,HPV DNA combined with TCT,and HPV E6/E7 mRNA combined with TCT in the ROC curve was 0.777,0.728, and 0.839, respectively. The AUC value of HPV E6/E7 mRNA combined with TCT was the highest, indicating that HPV E6/E7 mRNA combined with TCT had the greatest diagnostic value for CIN Ⅰ progression.Conclusion The HPV E6/E7 mRNA test has certain significance in predicting the outcome of CIN Ⅰ,which can be used for the follow-up shunt management of CIN Ⅰ,and is worthy of clinical promotion and application.

    • Clinical characteristics and therapeutic efficacy analysis of primary bilateral macronodular adrenal hyperplasia

      Abstract:Objective To analyze the clinical characteristics and the efficacy of different surgical approaches of patients with primary bilateral macronodular adrenal hyperplasia(PBMAH).Methods Clinical data of 119 patients diagnosed with adrenocorticotropic hormone(ACTH)-independent Cushing’s syndrome(CS) due to unilateral adrenal adenoma(UAA) and 34 patients with PBMAH in our hospital from January 2011 to August 2021 were analyzed retrospectively. Their clinical data such as clinical features,metabolic-related diseases,hormone secretion levels and imaging features were compared to summarize the clinical features of PBMAH. At the same time,the changes in clinical manifestations,metabolic diseases and hormone secretion levels of PBMAH patients before and after surgery were compared to to evaluate the therapeutic efficacy of different surgical procedures on PBMAH.Results Both PBMAH patients and UAA patients had significantly different clinical manifestations. The mean age at diagnosis was (49.8±13.1) years in the PBMAH group,which was significantly higher than that of the UAA group’s(43.0±11.6) years(P=0.016). The incidence of PBMAH group was similar in both genderes,while the UAA group was significantly higher in women than in men(P<0.001). Subclinical Cushing’s syndrome(SCS)(55.9%) was the main manifestation of PBMAH patients,and metabolic-related diseases such as hypertension and diabetes were the most common. In terms of hormone secretion levels,the cortisol level in the PBMAH group was significantly lower than that in the UAA group,especially serum cortisol at 16:00,24:00 and 24-hour urinary free cortisol. And 10 patients had a family history of genetic disorders,and were tested for mutations in the ARMC5 gene. Imaging showed diffuse large nodular hyperplasia of the adrenal glands bilaterally,whereas UAA patients had unilateral solitary adenomas. For treatment,PBMAH patients with SCS underwent unilateral adrenalectomy with a high clinical remission rate(100%),while PBMAH patients with Cushing’s appearance had a clinical remission rate of 50% after unilateral resection,and 16.7% of patients relapsed. And the incidence of adrenocortical insufficiency after unilateral adrenalectomy(28.6%) was significantly lower than that after bilateral adrenal surgery(100%).Conclusion PBMAH is a familial hereditary disease with a higher age of onset,often manifesting as SCS,with hypertension and diabetes as the first symptoms. PBMAH patients combined with SCS are recommended to undertake unilateral total adrenalectomy. Patients with obvious symptoms of CS are recommended to undertake total adrenalectomy on one side and major adrenalectomy on the opposite side.

    • Identification of high risk patients with preoperative low albumin for advanced gastric antrum cancer based on nomogram model

      Abstract:Objective To explore the high risk factors of hypoalbuminemia before radical gastrectomy,and influence on prognosis.Methods From June 2008 to June 2015,a total of 497 patients with gastric antrum cancer treated by surgery in Fujian Cancer Hospital were included in the study,then they were divided into low albumin group(< 35 g/L,194 cases)and normal albumin group(≥35 g/L,303 cases)according to the preoperative serum albumin level. The high risk factors of hypoalbuminemia before radical operation and its impact on prognosis were analyzed.Results The univariate study found that there were significant differences in age,tumor size,differentiation degree and tumor stage between hypoalbuminemia group and normal albuminemia group. The multivariate study found that age,tumor size,TNM stage were independent risk factors for preoperative hypoalbuminemia. The nomogram risk prediction model was established. The 1-year,3-year and 5-year survival rates of low albumin group were 96.3%,85.7% and 78.7% respectively. The 1-year,3-year and 5-year survival rates of normal albumin group were 94.1%,76.0% and 62.9% respectively(P<0.001). The results of univariate and multivariate analysis showed that age,T stage,TNM stage and preoperative albumin were independent prognostic factors for patients(P<0.05).Conclusion For patients with gastric antrum cancer,age,tumor size and TNM stage are independent risk factors of preoperative hypoalbuminemia. That is an indication of poor prognosis of patients with gastric antrum cancer. For these patients,identification and intervention measures should be strengthened.

      Abstract:Objective Molecular markers related to the prognosis of hepatocellular carcinoma are essential for improving the quality of life of patients and improving the therapeutic efficacy. This study aims to investigate the effect of cell division cycle associated 8 (CDCA8) on the prognosis of hepatocellular carcinoma and its potential molecular mechanism.Methods The transcriptome data of 432 cases of hepatocellular carcinoma and the clinical information of the corresponding patients were downloaded from The Cancer Genome Atlas (TCGA) database. The differences of CDCA8 mRNA in tumor tissues and normal tissues were analyzed. The patients with hepatocellular carcinoma were divided into high expression group and low expression group according to the median CDCA8 mRNA,and the survival curve was drawn. Kruskal test was used to analyze the correlation between CDCA8 and tumor stage,grade and T stage. Univariate and multivariate Cox regression analysis were used to investigate whether CDCA8 could be used as an independent prognostic factor for patients with hepatocellular carcinoma. The potential molecular mechanism of CDCA8 in hepatocellular carcinoma was explored using gene set enrichment analysis (GSEA).Results The mRNA level of CDCA8 in tumor tissues was significantly higher than that in control tissues (P<0.001),and the survival rate of patients with high CDCA8 expression was significantly lower than that of those with low expression (P<0.001). As tumor stage (P<0.001),grade (P<0.001) and T stage (P<0.001) increase,the degree of malignancy of hepatocellular carcinoma increases,accompanied by an increase of the mRNA level of CDCA8. GSEA revealed significant pathways for CDCA8 overexpression including cell cycle and mitosis.Conclusion CDCA8 can be used as a prognostic molecular marker for patients with hepatocellular carcinoma,and it is expected to become a new target after in-depth study.

    • Clinical analysis of antegrade dissection re-entry technique for coronary chronic occlusion lesion

      Abstract:Objective To investigate the efficacy and safety of antegrade dissection re-entry(ADR) technique in percutaneous coronary intervention (PCI) in coronary chronic total occlusion(CTO) lesions.Methods A total of 98 patients with CTO who received PCI treatment in Binzhou People's Hospital and The Second Affiliated Hospital of Nanchang University from January 2017 and December 2018 were included in the study. Among them,48 patients were given traditional PCI treatment without ADR technique(control group) and 50 patients were treated with ADR technique(treatment group). The baseline conditions, coronary angiography results,PCI success rate,and major adverse cardiovascular events(MACE) for 12 months of follow-up were compared between the two groups.Results There were 52 CTO lesions in 48 patients in the control group and 58 ones in 50 patients in the treatment group. The success rate of PCI in the treatment group was significantly higher than that in the control group(89.7% vs. 71.2%,P=0.047),in which,6 cases of CTO due to in-stent restenosis(ISR-CTO) in treatment group were all open. The mean PCI time [(71±25) min vs. (95±33) min,P=0.041],X-ray exposure time [(42±17) min vs. (71±22) min,P=0.032] and contrast dose [(98±26) mL vs. (178±63) mL,P=0.029] in the treatment group were significantly lower than those in the control group,and all the differences were statistically significant. The incidence of MACE(22.0% vs. 41.7%, P=0.046) and recurrent myocardial infarction(10.0% vs. 27.1%,P=0.047) after 12 months of follow-up were significantly lower in the treatment group than in the control group,and the differences were statistically significant.Conclusion The application of ADR in coronary CTO lesion PCI is safe and effective,which shortens the operation time,reduces the radiation dose and contrast dose of doctors and patients,and improves the prognosis of patients.

      Abstract:Objective To analyze the factors influencing the effect of wearing orthokeratology on controlling myopia in adolescents, and provide basis for clinical work.Methods A retrospective study was conducted among 105 cases of juvenile myopia(206 eyes) who came to the Wuhan Aier Eye Expert Hospital affiliated to Wuhan University from February to August,2019. The correlation between the parameters(age,gender,diopter,axial length,corneal astigmatism,flat k,steep k,eccentricity value,pupil diameter, corneal diameter,position) and ocular axial length changes after wearing orthokeratology for 18 months was analyzed. SPSS 23.0 was used for generalized linear model fitting and one-way ANOVA.Results Backward selection multivariate analysis showed that the ocular axial length changes were correlated with the age and original diopter(P<0.001,P<0.001). The older the patient’s baseline age,the slower the axial length growth. The one-way ANVOA showed that the effect of myopia above -3.00 D was better than that below -3.00 D(P<0.01),showing no significant difference between the two groups.Conclusion Age and diopter are influencing factors of wearing orthokeratology to control juvenile myopia. The axial growth of the older patients is slower than that of the younger patients,so each area of the orthokeratology should be designed individually for the younger patients. The effect is better on myopia above -3.00 D than that on myopia below -3.00 D. However,the control effect of myopia less than 3.00 D and greater than 3.00 D is not affected by the increase or decrease of diopter. Patients with myopia below -3.00 D should be closely followed up and treated combined with other myopia control methods.

      Abstract:Objective To explore the association of triglycerides(TG)total cholesterol(TC)body weight(BW)index(TG×TC×BW index,TCBI)with carotid artery disease.Methods A total of 2 290 patients with suspected stable carotid artery disease who were hospitalized in a hospital of Ma'anshan City,Anhui Province from November 2016 to October 2019 were selected as the research subjects. All of them were divided into carotid artery disease group(n=1 352)and control group(n=938)according to the results of color Doppler detection. We collected the general demographic characteristics,clinical characteristics,color Doppler examination indexes and blood biochemical detection indexes. Regression models were established to analyze the relationship between TCBI and carotid artery disease.Results The TCBI of the carotid artery disease group(χ2=34.018,P<0.001)was higher than that of the control group;dyslipidemia(χ2=143.933,P<0.001)and diabetes(χ2=29.946,P<0.001,internal carotid artery resistance index(ICA-RI)(t=2.468,P=0.014),triglyceride glucose(TyG)index(t=9.160,P<0.001),intima-media thickness(IMT)≥ 1.0 mm(t=34.018,P<0.001)in the high TCBI group were higher than those of the low TCBI group. TCBI was positively correlated with dyslipidemia(r=0.510,P<0.001),ICA-RI(r=0.363,P<0.001and atherogenic index of plasma(r=0.714,P<0.001). In the correlation analysis,the TCBI>1 800 was an independent risk factor for carotid artery disease.Conclusion The TCBI has potential application value in the screening and clinical diagnosis of carotid artery disease.

      Abstract:Objective To investigate the sensitizing effect of SCR7 on cisplatin chemotherapy in triple-negative breast cancer(TNBC).Methods MDA-MB-231 cells of TNBC were selected and divided into blank control group,10 μmol/L cisplatin control group and combined group(10 μmol/L cisplatin + 10,20 and 40 μmol/L SCR7). CCK-8 method was used to detect cell activity and calculate cell proliferation inhibition rate. A TNBC mice model was constructed by using TNBC MDA-MB-231 cells. The nude mice were randomly divided into control group,SCR7 group,cisplatin group and SCR7+cisplatin group. After 24 days of feeding,the changes of mass and tumor volume of tumor-bearing mice in all groups were observed and compared,and the tumor inhibition rate was calculated. Morphological changes of tumor cells were observed by hematoxylin - eosin(HE) staining. The apoptosis of tumor cells was detected by in situ end labeling(TUNEL) method,and the expressions of X-ray repair cross complementing 1(XRCC1) and poly(ADP-ribose) polymerase 1(PARP1) proteins were detected by Western blot.Results The cell proliferation inhibition rate in the combined group was higher than that in the control group(P<0.05),and it wasgradually increased with the increase of SCR7 concentration(P<0.05). Animal experiments in vivo found:compared with the control group,the tumor volume and tumor weight of breast cancer in SCR7 group,cisplatin group and SCR7+cisplatin group decreased(P<0.05),and the tumor inhibition rates were 32%,40% and 81%,respectively. The SCR7+cisplatin group showed a more significant reduction than SCR7 group and cisplatin group(P<0.05),but there was no significant difference in mouse body weight compared to the cisplatin group(P>0.05). The tumor tissues in the intervention groups showed obvious morphological changes by HE staining,among which the tumor cells in the SCR7+cisplatin group showed the most obvious degenerative changes. TUNEL staining showed that the tumor cells in the SCR7+cisplatin group had the most serious apoptosis,and a large number of red apoptotic cells were seened. Western blotting showed that the protein expressions of XRCC1 and PARP1 in SCR7+cisplatin group were significantly lower than those in other groups,and the differences were statistically significant(P<0.05).Conclusion SCR7 has sensitizing effect on cisplatin chemotherapy for TNBC in vivo and in vitro.

      Abstract:With the increase of the proportion of obese population,the incidence of obesity related metabolic diseases such as nonalcoholic fatty liver disease(NAFLD) and type 2 diabetes mellitus(T2DM) is increasing year by year,which brings a heavy burden to the family and society. A large number of experimental data confirm that intestinal barrier injury plays an important role in the occurrence and development of obesity related metabolic diseases. This paper summarizes the structure,function and evaluation methods of intestinal barrier in detail,and summarizes the research progress of intestinal barrier injury and obesity related metabolic diseases,so as to provide a theoretical basis for how to evaluate patients' intestinal barrier function and how to treat metabolic diseases by improving intestinal barrier function.

    • Mechanisms of mitochondrial Drp1 inducing excessive opening of mPTP channel through LRRK2-HK2 after hypoxia

      Abstract:Objective To explore the regulatory mechanisms of mitochondrial dynamin-related protein 1(Drp1) on excessive opening of mitochondrial permeability transition pore(mPTP) after hypoxia.Methods The opening of mPTP after hypoxia was observed by immunofluorescence in vascular smooth muscle cells(VSMCs). Mitochondrial and cytoplasmic fractions were separated by ultracentrifugation. Co-IP and Western blot methods were used to detect the expression and distribution of Drp1,the binding of hexokinase-2(HK2) to voltage dependent anion channel(VDAC),and the binding of Drp1 to leucine-rich repeat kinase 2(LRRK2). The potential binding proteins and binding sites of Drp1 were screened by protein molecular docking and protein chip. Mdivi-1,a Drp1 inhibitor,and mutation methods were used to explore the corresponding mechanisms.Results Mitochondrial translocation of Drp1 caused mPTP excessive opening in VSMCs after hypoxia(P<0.05). Reducing mitochondrial Drp1 expression by Mdivi-1 could inhibit mPTP opening and reduce the release of cytochrome C(CytC)(P<0.05). The mitochondrial separation of HK2 caused by the decrease of HK2-Thr473 phosphorylation after hypoxia leaded to the destruction of mPTP structure. After restoring HK2 activity by HK2 T473D mutation,the binding of HK2 to mitochondria and the opening of mPTP after hypoxia were significantly improved(P<0.05). The results of Drp1 protein chip showed that Drp1 could bind to kinase LRRK2 and block its active site after hypoxia. After the combination of Drp1-LRRK2 was broken by the site mutation of Drp1 T595A,the activity of HK2 and the mitochondrial separation of HK2 were obviously improved(P<0.05),and the opening of mPTP channel was obviously reduced(P<0.05).Conclusion Mitochondrial Drp1 may reduce HK2-Thr473 phosphorylation and cause HK2 mitochondrial separation by binding and blocking the active site of kinase LRRK2,which finally induces the excessive opening of mPTP after hypoxia.

    • Correlation between the ascending aortic elasticity evaluated by speckle tracking imaging and the coronary artery lesion of patients with coronary heart disease

      Abstract:Objective To evaluate the elastic function of ascending aortic in patients with coronary heart disease(CHD) by two-dimensional speckle tracking imaging(2D-STI),and to explore its correlation with the degree of coronary artery lesion.Methods A total of 206 patients with suspected CHD who were admitted to our hospital due to chest tiredness or chest pain from January 2020 to March 2021 were enrolled in the study. All subjects underwent echocardiography,and routine arterial stiffness indexes such as the ascending aortic distensibility(AoDIS),ascending aortic stiffness index(AoSI) and ascending aortic strain values were calculated. Then 2D-STI was used to measure the global circumferential ascending aortic peak strain(GCAAS) and circumferential ascending aortic peak strain rate(CAASR). According to the results of coronary angiography and Gensini score(GS),they were divided into CAD group(mild group,moderate group and severe group) and control group.Results ①AoDIS,ascending aortic strain value,GCAAS and CAASR in the CHD group were lower than those in the control group,and AoSI in the CHD group was higher than that in the control group(all P<0.05). ②Univariate logistic regression analysis showed that man,diabetes,hypertension,left ventricular ejection fraction (LVEF),AoDIS,GCAAS and CAASR were all risk factors for CHD(all P<0.05). After multivariate analysis,GCAAS was still a risk factor for CHD(P<0.001). ③In the CHD group,GCAAS and CAASR decreased as the severity of coronary artery lesion increased(P=0.003,P=0.015). GCAAS and CAASR in the severe group were significantly lower than those in the mild group(P=0.001,P=0.012). Spearman correlation analysis showed that GCAAS was moderately correlated with GS(rs=-0.411,P<0.001),while CAASR was weakly correlated with GS(rs=-0.228,P=0.027).Conclusion The ascending aortic strain of 2D-STI is superior to conventional ultrasound technology in reflecting the early changes in the elasticity of the ascending aortic in patients with CHD. The global circumferential ascending aortic strain is closely related to the occurrence and progression of CHD and the severity of coronary artery lesion.

    • Effects of laparoscopic partial salpingectomy followed by end-to-end reanastomosis versus salpingotomy on tubal recanalization and fertility prognosis in women with tubal pregnancy

      Abstract:Objective To investigate the therapeutic effect of laparoscopic partial salpingectomy followed by end-to-end reanastomosis versus salpingotomy on tubal recanalization and fertility prognosis in women with tubal pregnancy.Methods A total of 98 patients with tubal pregnancy with fertility expectation and risk of fertility decline from April 2018 to April 2021 were prospectively included in the study. They were randomly divided into salpingectomy & anastomosis group(n=50) and salpingotomy & suture group(n=48). The salpingectomy & anastomosis group underwent laparoscopic partial salpingectomy and end-to-end anastomosis. The fallopian tube was retrogradely intubated(the epidural anesthesia catheter was inserted from the umbrella end of the fallopian tube and ran through the two broken ends of the fallopian tube),which was used as a support to assist the anastomosis and hydrotubation during the operation in the salpingectomy & anastomosis group. The salpingotomy & suture group underwent laparoscopic salpingotomy and embryo suture,and 50 mg of methotrexate was routinely injected into the mesangium near the tubal pregnancy sac. The two groups had routine contraception for 3 months after the operation,and salpingography was performed 3 months after operation to observe the patency of fallopian tubes. The operation time,intraoperative bleeding,the incidence of postoperative persistent ectopic pregnancy and the patency of fallopian tubes 3 months after the operation were compared between the two groups. The patients were followed up to observe the occurrence of intrauterine pregnancy and re-tubal pregnancy in the two groups.Results The salpingectomy & anastomosis group had a significantly longer operation time [(52.3±5.7) min vs. (48.6±5.5) min,P=0.002],less amount of intraoperative bleeding [(6.1±3.9) mL vs.(9.6 ± 5.7) mL,P=0.000],and lower incidence of postoperative persistent ectopic pregnancy(0.0% vs. 14.6%,P=0.016)than the salpingotomy & suture group,with statistical significance(P<0.05). Three months after operation,the recanalization rate of fallopian tube was significantly higher in the salpingectomy & anastomosis group(94.0% vs. 79.2%,P=0.030) than the salpingotomy & suture group. All patients were followed up. After(18.5±5.7) months of follow-up,the intrauterine pregnancy rate in the salpingectomy & anastomosis group was significantly higher(58.0% vs. 37.5%,P=0.042),and the incidence of re-tubal pregnancy was significantly lower(2.0% vs. 16.7%,P=0.031) than those in the salpingotomy & suture group respectively. There was no significant difference in the incidence of adverse outcomes of intrauterine pregnancy between the two groups.Conclusion Laparoscopic partial salpingectomy and end-to-end anastomosis is safe and reliable in the treatment of tubal pregnancy,which can reduce the risk of postoperative persistent ectopic pregnancy and re-tubal pregnancy,and improve the postoperative tubal recanalization rate and intrauterine pregnancy rate.

    • Advances in the detection of RNA epigenetic modifications

      Abstract:RNA modification is an important factor in the post-transcriptional regulation of gene expression and is of great research importance in all areas of epigenetics. Currently,many known methods for RNA modification detection rely in part on the specificity of antibodies. The most commonly used method is high-throughput sequencing based on antibody immunoprecipitation,but this method is also prone to false-positive and false-negative results. Therefore,a series of antibody-free sequencing methods have also emerged in recent years,as well as the exploration of new methods for cost-effective and simple antibody-free RNA modification biosensing assays. In addition,site-specific RNA modification detection is crucial for probing the specific regulatory mechanisms of methylation on diseases,and single-base resolution assay construction is a necessary prerequisite. This review summarizes new methods for the specific detection of RNA methylation modifications in recent years and for the quantitative detection of site-specific RNA modifications,and mainly summarize the advantages and shortcomings of different detection methods based on immune pathways,methylation-specific enzyme pathways,hybridization methods,chemical treatment,labeling techniques,and in situ detection of RNA modifications. We also discuss the research prospects of universal detection methods and third-generation sequencing for direct detection of RNA methylation modifications.
